- present a completed "Public Health Passenger locator form"; and
- have a travel insurance; and
- complete an "ETIS Application" at https://www.etis.pf/en/ . A QR code generated from the completed form must be presented upon arrival.

Passengers must complete a declaration on honor to state that they do not have symptoms of Coronavirus (COVID-19). The declaration must be dated with the name and signature of the passenger.
