Passengers arriving from Andorra, Austria, Belgium, Bulgaria, Croatia, Cyprus, Denmark, Estonia, Germany, Hungary, Italy, Latvia, Liechtenstein, Lithuania, Luxembourg, Monaco, Netherlands, Poland, Romania, San Marino, Slovakia, Slovenia, Sweden, Switzerland or United Kingdom must complete a "Passenger Locator Form " no later than on arrival in Czechia.
